Job Description:
As a Transition Adult Social Worker, you will support individuals aged 14 to adulthood, facilitating their transition into adult services.
This role is a fantastic opportunity for social workers experienced in children's services to progress into adult social work while maintaining a focus on safeguarding, independence, and well-being.

Responsibilities:
- Conduct assessments for young people transitioning into adult services, focusing on their needs, risks, and support plans.
- Develop and implement person-centered plans to promote independence and well-being.
- Collaborate with families, carers, and multi-agency teams to provide comprehensive support.
- Monitor and review care plans, ensuring they adapt to the changing needs of the individual.
- Provide advocacy for young people, ensuring their voices are heard throughout the transition process.
- Maintain accurate and up-to-date case records, adhering to statutory and professional standards.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Experience working in children's services or adult social care, particularly with transitions.
- Strong understanding of relevant legislation, including the Care Act and Children and Families Act.
- Excellent communication and relationship-building skills.
- Ability to work independently, manage a caseload, and make informed decisions under pressure.
