Middlesbrough Council's Adult Social Care is seeking a dedicated and skilled Adult Social Worker to join our dynamic Transformation East and West Locality Team.
Your Role:
As a Social Worker within our Transformation Team, you will play a vital role in shaping the future of our services. You will work with a dedicated team, contributing to service improvement projects, implementing new approaches, and providing high-quality support to adults and their families.

... - Direct Work with Adults: Building strong and trusting relationships with individuals and their families, providing them with the necessary support, guidance, and interventions.
- Case Management: Taking responsibility for a caseload of individuals, conducting thorough assessments, developing individualized care plans, and ensuring their needs are met.
- Promoting Wellbeing: Working with individuals and families to address their needs, reduce risk factors, and promote positive outcomes.
- Multi-agency Collaboration: Collaborating effectively with other professionals and agencies involved in an individual's care, including health services, housing providers, and community support organizations.
- Advocacy and Support: Advocating for the needs and rights of individuals, ensuring they receive the appropriate support and resources.
- Promoting Positive Change: Encouraging positive change within individuals and their families, empowering them to make choices that support their wellbeing.
- Contribute to Service Improvement: Working alongside the team to implement new initiatives, refine existing services, and shape the future of adult social care services within the locality.
Key Requirements:
- Social Work Qualification: You must be a qualified Social Worker.
- Social Care England Registration: Essential for this role.
- Updated DBS: Essential for working with vulnerable individuals.
- Experience in Adult Social Care: Demonstrated experience working with adults experiencing difficulties, with a strong understanding of relevant legislation and policy frameworks.
- Excellent Assessment and Case Management Skills: Proven ability to conduct comprehensive assessments, develop effective care plans, and manage complex cases effectively.
- A Flexible and Adaptable Approach: Ability to embrace change, learn new practices, and contribute to a dynamic team environment.
