Responsibilities:
- Assessment and Intervention:
- Conduct thorough assessments of adults at risk, taking into account their physical, emotional, and social needs.
- Develop and implement individualised care plans to address identified issues and risks.
- Case Management:
- Manage a caseload of adult clients, ensuring regular and effective communication with clients, their families, and other stakeholders.
- Coordinate and collaborate with multi-disciplinary teams to provide holistic support.
- Crisis Intervention:
- Respond promptly to crises and emergencies involving vulnerable adults, implementing appropriate intervention strategies to ensure their safety.
- Advocacy:
- Advocate for the rights and needs of adults at risk, ensuring their voices are heard and respected in decision-making processes.
- Collaboration and Networking:
- Establish and maintain effective working relationships with external agencies, service providers, and community resources to enhance support services for clients.
- Documentation:
- Maintain accurate and up-to-date records of assessments, interventions, and case progress in accordance with organisational policies and legal requirements.
Qualifications:
- Bachelor's or Master's degree in Social Work
- Proven experience in adult protection and safeguarding.
- Strong knowledge of relevant legislation and regulations.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Compassion, empathy, and a commitment to promoting the well-being of vulnerable adults.
