About the Role:
Are you a dedicated and empathetic Social Worker passionate about supporting adults through significant life transitions? Rotherham Metropolitan Borough Council is seeking an experienced Adults Social Worker to join our proactive Transitions Team. This role is pivotal in ensuring that individuals moving from children's to adults' services receive seamless and person-centred support.
Key Responsibilities:
- Work with young adults and their families to navigate the transition from children's to adults' social care services, ensuring continuity of care and support.
- Conduct comprehensive assessments to identify individual needs and develop tailored transition plans.
- Collaborate with multi-disciplinary teams, including education, health, and housing professionals, to provide integrated and holistic support.
- Advocate for the rights and needs of young adults, ensuring their voices are central to the planning and delivery of services.
- Maintain up-to-date and accurate records in line with statutory requirements and best practice guidelines.
- Provide timely interventions and support, addressing any issues that arise during the transition process.
- Stay informed about relevant policies, legislation, and local resources to effectively support individuals and their families.
About You:
- Qualified Social Worker with valid Social Work England registration.
- Significant experience in working with young adults, particularly in a transitions or similar role.
- Strong assessment, planning, and analytical skills.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with young adults, families, and professionals.
- A proactive and solution-focused approach to problem-solving.
- In-depth knowledge of relevant legislation, policies, and best practices in adult social care and transitions.
- Commitment to continuous professional development and reflective practice.
