About the Role
As a Social Worker in our Children's Long Term Team, you will be responsible for working with a caseload of children and young people with a variety of complex needs, including those with disabilities, chronic health conditions, and those in the care system. You will:
- Conduct thorough assessments of children's needs and their families, identifying strengths, challenges, and goals.
- Develop and implement individualised care plans, ensuring tailored support is provided across different areas, including education, health, social care, and family well-being.
- Work closely with families, providing guidance, support, and advocacy to navigate the complexities of their child's needs.
- Collaborate effectively with a wide range of professionals, including health professionals, educators, and other agencies, to create a cohesive and integrated support network.
- Ensure the child's voice is heard and their rights are protected, advocating for their needs and promoting their best interests.
- Maintain accurate and up-to-date records, adhering to relevant legislation and policies.
Who we're looking for:
We are looking for a highly motivated and compassionate Social Worker with a strong understanding of the needs of children with long-term needs and their families.
You will have:
- A recognised social work qualification (e.g., DipSW, CQSW, BSC Social Work)
- A valid Social Work England registration
- Significant experience working with children with complex needs and their families
- Excellent communication and interpersonal skills, with the ability to build strong relationships and provide effective support
- A deep understanding of relevant legislation, policies, and safeguarding procedures
- A genuine commitment to child-centered practice and promoting the well-being of children
