We are seeking an experienced Agency Social Worker to join the Leaving Care Team on a temporary contract. In this vital role, you will work directly with young people aged 16 to 25 who are transitioning out of care, providing essential support and guidance to help them achieve independence and stability.

Key responsibilities will include:

• Conducting comprehensive assessments and developing individual support plans for young people in the leaving care process.
• Providing practical and emotional support to young people as they transition to independent living, including assistance with housing, education, employment, and financial management.
• Supporting young people in developing essential life skills and building resilience to navigate adulthood successfully.
• Collaborating with multi-agency partners, including education providers, housing services, and health professionals, to ensure holistic support for young people.
• Regularly reviewing and monitoring progress against support plans, making necessary adjustments to ensure the best outcomes for each young person.
• Advocating for young people's rights and needs within the care system and beyond.

Requirements:

• Qualified Social Worker with current registration with Social Care Wales.
