Job Description:
As an Adults Learning Disability Social Worker, you will play a crucial role in supporting adults with learning disabilities to achieve their fullest potential. You will work collaboratively with individuals, families, and multidisciplinary teams to provide tailored support plans that promote independence, well-being, and social inclusion.
Key Responsibilities:
- Conduct comprehensive assessments to identify the needs of adults with learning disabilities.
- Develop, implement, and review personalised care plans in collaboration with service users, families, and other professionals.
- Advocate for the rights and needs of individuals with learning disabilities, ensuring their voice is heard and respected.
- Provide information, advice, and support to service users and their families regarding available services and resources.
- Collaborate with a range of professionals and agencies to coordinate care and support services.
- Maintain accurate and up-to-date records in accordance with legal and organisational requirements.
- Participate in regular supervision and continuous professional development to enhance skills and knowledge.
Requirements:
- Degree in Social Work and current registration with Social Work England.
- Experience working with adults with learning disabilities is essential.
- Strong understanding of the legislative framework and best practices in social care for adults with learning disabilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong assessment, planning, and organisational skills.
