As a Social Worker in the Initial Assessment Team, you will be pivotal in the first response to referrals involving vulnerable adults and families. Your role will involve conducting detailed initial assessments to determine immediate needs, risks, and appropriate interventions. You will ensure that service users receive timely and effective support while safeguarding their well-being in accordance with statutory requirements and council policies.
Responsibilities include:
... - Undertake thorough initial assessments of individuals and families referred to the council, identifying immediate needs and risks
- Handle urgent and complex cases with sensitivity and efficiency, providing crisis intervention and support as needed
- Make well-informed recommendations based on assessment outcomes, which may include referrals for long-term services, intervention plans, or safeguarding actions
- Determine an appropriate level of intervention and service provision
Requirements
- Relevant social work qualification
- 2 experience in adult social work, including assessment and care planning.
