Job Description:
Are you a compassionate and experienced social worker looking to support patients during their critical times?
In this role, you will be responsible for assessing patients' needs, developing discharge plans, and coordinating care to ensure a smooth transition from hospital to home or other care facilities.
Responsibilities:
- Conducting comprehensive assessments of patients' needs, risks, and strengths.
- Developing and implementing discharge plans that ensure patients receive appropriate care and support post-hospitalization.
- Coordinating with healthcare professionals, community organizations, and other relevant agencies to ensure a seamless transition for patients.
- Providing direct support and intervention to patients and their families, ensuring access to necessary services and resources.
- Monitoring and reviewing care plans regularly, making adjustments as necessary to respond to changing needs and circumstances.
- Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Proven experience working in a hospital or healthcare setting.
- Excellent communication and interpersonal skills, with the ability to build trusting relationships with patients, families, and healthcare partners.
- Knowledge of relevant legislation, policies, and procedures related to healthcare and social care.
