Are you a compassionate and experienced social worker looking to make a difference from the first point of contact?
In this role, you will be responsible for conducting initial assessments, providing urgent support, and referring individuals and families to appropriate services.

Responsibilities:

• Conducting thorough initial assessments of individuals and families to identify needs, risks, and strengths.
• Providing immediate support and intervention to address urgent needs and stabilize situations.
• Collaborating with multi-disciplinary teams, external agencies, and community organizations to coordinate comprehensive care and support.
• Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.
• Ensuring compliance with relevant legislation, policies, and procedures.

Qualifications:

• Qualified Social Worker registered with Social Work England.
• Proven experience working in a social care setting, preferably with initial assessments and urgent support.
• Excellent communication and interpersonal skills, with the ability to build trusting relationships with individuals and families.
• Knowledge of relevant legislation, policies, and procedures related to social care and safeguarding.
