Doncaster Metropolitan Borough Council is seeking a dedicated and compassionate Social Worker to join our Children with Disabilities Team. This is a unique opportunity to make a significant impact in the lives of children with disabilities and their families, providing essential support and services tailored to their needs.
Key Responsibilities:
... Assessment and Planning:
- Conduct comprehensive assessments of children with disabilities to determine their needs and develop tailored care plans.
- Work collaboratively with children, families, and multi-disciplinary teams to create effective support plans.
- Regularly review and update care plans to ensure they remain relevant and effective.
Support and Intervention:
- Provide direct support and intervention to children with disabilities and their families, promoting their well-being and safeguarding.
- Offer emotional and practical support, helping families navigate available services and resources.
- Implement interventions that promote independence, social inclusion, and improved quality of life for children with disabilities.
Advocacy and Empowerment:
- Advocate for the rights and needs of children with disabilities, ensuring their voices are heard in the planning and delivery of their care.
- Empower families by providing information, guidance, and support to help them make informed decisions.
Collaboration and Partnership:
- Work closely with other professionals, including health, education, and voluntary sector partners, to provide integrated and holistic support.
- Attend and contribute to multi-disciplinary meetings, case conferences, and reviews.
Record Keeping and Compliance:
- Maintain accurate and up-to-date case records in compliance with statutory requirements and best practice guidelines.
- Ensure all work is carried out in accordance with relevant legislation, policies, and procedures.
About You:
Qualifications:
- Qualified Social Worker with a valid Social Work England registration.
Experience:
- Proven experience in working with children with disabilities and their families.
- Strong understanding of the challenges faced by children with disabilities and knowledge of relevant support services and resources.
Skills and Attributes:
- Excellent assessment, planning, and organisational skills.
- Strong communication and interpersonal skills, with the ability to build trusting relationships with children, families, and professionals.
- Empathetic, patient, and resilient, with a commitment to promoting the best interests of children with disabilities.
- Ability to work effectively as part of a team and independently.
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
