Position Overview: We are currently seeking a motivated and dedicated Social Worker to join our Adult Review Team on a 6-month contract basis. As a member of the Adult Review Team, you will play a crucial role in conducting assessments, reviewing care plans, and coordinating services to ensure the best possible outcomes for adults in need of support.
Key Responsibilities:
- Conducting comprehensive assessments of adults in need of care and support.
- Reviewing and updating care plans to reflect changing needs and circumstances.
- Coordinating with multi-disciplinary teams to deliver integrated and person-centered support services.
- Providing advocacy and support to ensure that adults receive the services and assistance they require.
- Liaising with external agencies, including healthcare providers and community organizations, to access additional support and resources.
- Monitoring and reviewing the effectiveness of support plans and making adjustments as necessary.
- Maintaining accurate and up-to-date records of assessments, interventions, and outcomes.
Qualifications and Experience:
- Qualified Social Worker with relevant experience in adult social care.
- Registration with Social Work England or equivalent regulatory body.
- Knowledge of relevant legislation, policies, and procedures related to adult social care.
- Strong assessment, communication, and interpersonal skills.
- Ability to work collaboratively as part of a multi-disciplinary team.
- Commitment to promoting the rights, dignity, and well-being of adults in need of support.
