We are seeking a dedicated and experienced Locality Social Worker to join our Adult Services team for one of our clients. The ideal candidate will have a strong background in social work, excellent case management skills, and a passion for supporting adults within the community. This role involves conducting assessments, developing care plans, and working collaboratively with multidisciplinary teams to ensure the best outcomes for service users. £33per hour hybrid working available.
Key Responsibilities:
- Conduct comprehensive assessments of adults' needs within the community.
- Develop, implement, and review personalized care plans in collaboration with service users and their families.
- Regularly monitor and adjust care plans to ensure they meet the evolving needs of service users.
- Manage a diverse caseload of adults with varying needs, ensuring each individual receives appropriate care and support.
- Maintain accurate and up-to-date case records, documentation, and reports.
- Coordinate and facilitate access to services and resources, such as healthcare, housing, and financial assistance.
- Provide emotional and practical support to adults, promoting their independence and wellbeing.
- Advocate for the rights and needs of service users within the community and healthcare system.
- Assist service users in navigating and accessing community resources and services.
Education: Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution.
Licensure: Reistered with Social Work England.
