Responsibilities: As a Localities Social Worker, your responsibilities will include:

🌟 Providing ongoing support and intervention to individuals and their families, ensuring their safety, well-being, and access to appropriate services.
🌟 Collaborating with multi-disciplinary teams, including healthcare professionals, support workers, and community organizations, to coordinate and deliver comprehensive care packages.
🌟 Advocating for the rights and interests of adults in need, promoting their autonomy and ensuring their voices are heard in decision-making processes.
🌟 Maintaining accurate and up-to-date records, documenting assessments, interventions, and outcomes in line with professional standards and legal requirements.
🌟 Participating in team meetings, case conferences, and training sessions to enhance your knowledge and skills and contribute to the continuous improvement of service delivery.
Qualifications and Experience: To excel in this role, you should possess:
🌟 A recognized qualification in Social Work and current registration with Social Work England.
🌟 Proven experience in adult services, with a solid understanding of relevant legislation, policies, and procedures.
🌟 Excellent assessment and analytical skills, enabling you to make informed decisions and manage complex caseloads effectively.
