This role requires you to work with adults undertaking strength based assessments. You must have good understanding of Community care Act and Mental Capacity Act as well as other social care legislation. You must be able to work in a person centred way and aim to help people to maintain their independence as well as recognising safeguarding.
Key Responsibilities:
Comprehensive Assessments:
- Conduct detailed assessments of adults referred to the service to identify their needs, strengths, and risks.
- Gather and analyse information from various sources, including interviews, home visits, and reports from other professionals.
- Evaluate the social, psychological, and environmental factors affecting the individual.
Care Planning:
- Develop, implement, and review individualised care plans in collaboration with adults and their families or carers.
- Ensure that care plans are tailored to meet the specific needs of each individual, promoting their independence and well-being.
- Regularly monitor and adjust care plans to reflect changes in circumstances or needs.
Intervention and Support:
- Provide direct support and intervention to adults in need, ensuring their safety and well-being.
- Assist individuals in accessing appropriate services and community resources.
- Offer guidance and advocacy to help individuals navigate social and healthcare systems.
Collaboration:
- Work closely with other professionals, including healthcare providers, housing services, and community organisations, to coordinate comprehensive care.
- Participate in multi-disciplinary meetings, case conferences, and planning sessions to ensure a holistic approach to support.
- Foster strong working relationships with all stakeholders to enhance service delivery.
Documentation and Reporting:
- Maintain accurate and up-to-date records of assessments, care plans, and interventions.
- Prepare detailed reports for case reviews, care planning meetings, and other relevant processes.
- Ensure compliance with all legal, regulatory, and organisational requirements.
Advocacy:
- Advocate for the rights and needs of adults within the care system and in other settings such as healthcare and housing.
- Ensure that the voices of individuals are heard and considered in all decision-making processes.
Continuous Improvement:
- Stay updated on best practices, legislation, and policies related to adult social care.
- Participate in training and professional development opportunities.
- Contribute to the continuous improvement of assessment services within the organisation.
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
-Eligibility to work in the UK
