We are seeking an experienced social care coordinator to join our team and provide essential aid to the health-care trust based in Belfast. The ideal candidate will actively seek to establish an empowered relationship with service users and their carers at all times through their actions which demonstrate dignity, compassion and respect.
LOCATION: Belfast (community based)
POSITION: Social Care Coordinator (Band 4)
PAY RATE: Band 4 £12.86
HOURS: Full time

Requirements:
- Minimum 3 years paid experience working with older people or people with physical disabilities within a hospital or care setting essential
- Must be currently NISCC registered
- Must hold a full car driving license with access to your own vehicle
- Knowledge of health care/ medical/ public sector highly desirable
- Ability to work flexibly
- Ability to work as part of a team
- Excellent organisational skills with an ability to manage workload
- Effective communication skills
- Confidence in data entry and record keeping
- Ability to demonstrate independent working
- Minimum 5 GCSE's inclusive of math and English (or equivalent) OR NVQ level 3 care (or equivalent)
Responsibilities:
- Provide a person centered social care service to older people, their families and their carers
- Carry out a range of activities and interventions in accordance with individual care plans to maintain well-being
- Monitor effectiveness of domiciliary care packages and services to support users living in their own home
- Build knowledge of community and voluntary networks and use resources to enhance well-being and quality of life
- Liaise with key staff in respect of appropriate care and support commissioned on behalf of service users
- Use information obtained from monitoring activities to inform a professional review
- Support responsible risk taking to contribute to the identification of risk
- Be alert and promptly report to indicators of change needed in order to prevent unnecessary deterioration or risk of harm
- Promote human rights of service users
- Support the social worker to comply with administrative processes around commissioning and monitoring of packages of care as agreed
- Ensure compliance within social care operating standards
- Take responsibility for own personal development and contribute to training as required
- Behave in a manner that reflects trust values
- Commitment to regular attendance and efficient completion of all tasks allocated
- Maintain high standards of personal responsibility
- Comply with the HPSS code of conduct
