As a Social Worker in the Adult Assessment and Safeguarding Team, you will be at the forefront of assessing the needs of vulnerable adults and ensuring their safety and well-being. Your role is vital in identifying risks, implementing safeguarding measures, and developing tailored care plans. Working closely with individuals, their families, and multidisciplinary teams, you will ensure that adults receive the right support, while promoting independence and maintaining their dignity.

Responsibilities include:
- Conduct comprehensive assessments under the Care Act 2014 to identify needs, risks, and eligibility for services, focusing on vulnerable adults and those at risk of harm.
- Identify and address safeguarding concerns, ensuring timely intervention to protect adults from abuse, neglect, or exploitation.
- Develop and implement person-centered care plans that promote independence, enhance well-being, and ensure safety.
- Collaborate with health professionals, housing services, community organisations, and other stakeholders to provide coordinated, multi-agency responses.
- Lead safeguarding investigations, implementing appropriate protection plans and following up to ensure ongoing safety and risk management.
- Regularly review and update care and safeguarding plans to adapt to any changing needs or circumstances.
- Advocate for the rights, well-being, and dignity of vulnerable adults, ensuring they have access to necessary services, resources, and support networks.
- Maintain compliance with statutory requirements, including safeguarding procedures, and adhere to best practice in risk management and adult protection.
Requirements:
- 2 years experience post qualification
- Action safeguarding procedures
