Your Role:

As a Social Worker in the REaCH Reablement Service, you will:

• Conduct holistic assessments to identify individual needs and strengths.
• Develop and implement reablement plans that promote independence and well-being.
• Work collaboratively with occupational therapists, support workers, and other professionals.
• Safeguard vulnerable adults by identifying and mitigating risks.
• Empower individuals to achieve their goals through a strengths-based approach.

What We're Looking For:

• A qualified and registered Social Worker with Social Work England.
• Experience in adult social care, reablement, or similar services.
• Strong assessment, planning, and intervention skills.
• A reflective and resilient approach to managing complex situations.
