Responsibilities:
- Coordinate the discharge planning process for patients, ensuring timely and appropriate discharges.
- Collaborate with healthcare professionals, including physicians, nurses, social workers, and therapists, to develop comprehensive discharge plans tailored to each patient's needs.
- Assess patients' post-discharge requirements, including home care services, medical equipment, and medication management.
- Communicate discharge plans and instructions to patients, families, and caregivers, ensuring they understand and are prepared for the transition.
- Coordinate transportation arrangements for patients as needed.
- Provide support and assistance to patients and families in navigating the post-discharge healthcare system.
- Ensure all discharge documentation and administrative tasks are completed accurately and promptly.
Requirements:
- Social Work England registered
- Social Work Degree or equivalent
- Enhanced DBS
