As a Social Worker in the Adult Generic Team, you will be crucial in assessing the needs of vulnerable adults and supporting them to maintain their independence and well-being. Your role will involve conducting person-centred assessments, developing care plans, and coordinating services to meet a wide range of needs, including physical health, mental well-being, and social support. You will collaborate closely with adults, their families, and other professionals to ensure they receive comprehensive care and support within the community.
Responsibilities include:
... - Conduct holistic assessments in accordance with the Care Act 2014, evaluating physical, mental, and social needs to formulate comprehensive, personalised care plans.
- Design and implement strengths-based care plans that promote autonomy, enhance well-being, and address complex needs across domains such as housing, healthcare, and social care.
- Deliver early intervention and preventative services, reducing the risk of crises or deterioration and enabling adults to maintain independence in their homes and communities.
- Collaborate with multidisciplinary teams (MDTs) including healthcare professionals, housing authorities, and community organisations to ensure integrated care pathways and coordinated service delivery.
- Identify and manage safeguarding concerns, implementing protective measures and adhering to Safeguarding Adults protocols to prevent abuse, neglect, or harm.
- Conduct ongoing care plan reviews, using data and feedback to adapt strategies to changing needs, ensuring ongoing effectiveness and compliance with statutory frameworks.
Requirements:
- 2 years experience post qualification
- Action safeguarding procedures
