What is on offer?
- Earn up to £45,416, including a 15% market supplement.

Responsibilities include:
- Assess and prioritise referrals to determine the urgency and appropriate course of action for each case, ensuring that children and families in need receive timely support.
- Act as an advocate for people, ensuring their voices are heard in decision-making processes and that they have access to the services and resources they need.
- Regularly monitor and review progress against transition plans, adjusting support as needed to ensure young people are on track to achieve their goals.
- Maintain accurate and up-to-date records of assessments, plans, and interventions, ensuring compliance with relevant legislation and policies.
Requirements:
- Relevant social work qualification
- 2 years relevant experience
