Responsibilities include:
- Conduct thorough assessments of adults' needs under the Care Act 2014 and other relevant legislation.
- Develop and implement personalized care plans aimed at promoting independence and community living.
- Safeguard vulnerable adults, ensuring their safety and well-being through appropriate interventions.
- Work collaboratively with multidisciplinary teams, families, and community services to provide holistic support.
- Advocate for adults' rights and ensure they have access to the necessary resources, services, and benefits.
- Regularly review and monitor care plans to adapt to changing needs and circumstances.
- Ensure compliance with statutory responsibilities and best practices in safeguarding and care provision.
Requirements:
- 2 years experience post qualification
- Action safeguarding procedures
