Responsible for:
- Carrying a caseload, undertaking holistic, strengths based assessments with adults, carers and families
- to help them make informed choices and decisions, enabling them to clarify and express their needs
- Undertaking person centred strengths based care act assessment to identify need taking the appropriate action whilst respecting individual values, cultures and diversity.
- During a strengths based assessment of need will assess and provide minor pieces of equipment or adaptations to support independence and reduce reliance on formal care to promote independence, prevent admission into hospital/residential care in order to manage transfers of care in a holistic, person centred way
- Providing written and verbal records and reports which are concise, factual and informative based on analysis of evidence which can be used within the legal context and where necessary give evidence in court in relation to care or other proceedings. (with support, guidance, joint work where necessary on complex cases)
- Identifying needs and create support plans to meet identified needs as well as determining eligibility for services in accordance with Care Act (2014) Guidance
- Assisting in the development and implementation of the locality based community led support model
- using creative ways to support service users and carers which promotes independence and reduce risks for Adults with Social Care needs living in Doncaster
- Undertaking appropriate case and risk management
- Supporting and encouraging people to utilise Community Led Support
- Providing peer support to colleagues and advice and guidance to Community Care Officers, new employees and student Social Workers
Minimum Requirements:
- SWE Registration
- 2 year post qualifying experience within UK
- Eligibility to work in the UK
