Immediate start with ongoing contract available with competitive rates up to £34.00 per hour
Responsibilities include:
-Managing a complex cases
-To identify the needs of clients and mobilise available resources as appropriate.
-To provide professional assistance and guidance to colleagues within the directorate and other agencies as appropriate.
-To ensure that records are kept in accordance with the policies and procedures of the directorate and other agencies.
-Undertaking social work assessments under the Care Act and undertaking MCA and BI work.
-Undertaking holistic assessment of individuals and their carers' needs, arranging and/or delivering services to meet these needs and reassessing needs as appropriate.
-Implement care plans and care packages
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
-Eligibility to work in the UK
