As a Social Worker in the Adult Services Team, you will work with adults requiring support, assessing their needs and providing person-centred care. Your role will involve conducting assessments under the Care Act 2014, developing personalised care plans, and facilitating access to services that promote independence, well-being, and long-term support. You will collaborate closely with service users, their families, and multidisciplinary teams to ensure their needs are met while adhering to statutory requirements and safeguarding procedures.
Responsibilities include:
... - Monitor, review, and maintain up-to-date support plans, ensuring they meet the changing needs of service users while delivering good value for money and advocating for their rights, choices, and access to services.
- Ensure that adults receive the appropriate support from the right professionals, tailored to their needs and promoting independence and well-being.
- Examine and oversee care strategies, ensuring that they are effective and sustainable.
- Support the assessment and care planning for high-cost placements and packages of care, ensuring appropriate resources are in place.
- Safeguard and promote the welfare of adults, adhering to relevant legislation and policies, including the Care Act 2014.
- Collaborate with service users, their families, and multidisciplinary teams to ensure seamless care and support.
- Maintain high standards of practice in line with statutory requirements and safeguarding procedures.
Requirements:
- 2 years experience post qualification
- Action safeguarding procedures
