đ©ș About the Role
As a Complex Care Nurse Case Manager, you'll take the lead in coordinating holistic care for individuals with chronic and complex conditions. Working closely with patients, families, commissioners, and multi-disciplinary teams, you'll ensure that care is delivered safely, compassionately, and effectively.
đ Key Responsibilities
đ Conduct full assessments of patient needs (clinical, social, and lifestyle)
đ§ Create and manage tailored, integrated care plans
đ€ Coordinate with GPs, therapists, social workers & external partners
đ Oversee care packages & monitor quality outcomes
đŁ Advocate for access to appropriate treatments, medications & support
đĄ Support smooth transitions across care settings (e.g., hospital discharges)
đŹ Educate patients and families in self-management & long-term care
đ Analyse trends & contribute to ongoing quality improvement
đĄ Stay up-to-date with evolving clinical guidelines and best practices
đ§ââïž Who We're Looking For:

Registered Nurse with a valid NMC PIN (LD or Paediatrics)
â
Experience in Paediatrics or disability complex care, case management, or community nursing
â
Compassionate, proactive and highly organised
â
Strong communication and leadership skills
