As a Complex Care Nurse Case Manager, you'll take the lead in coordinating holistic care for individuals with chronic and complex conditions. Working closely with patients, families, commissioners, and multi-disciplinary teams, you'll ensure that care is delivered safely, compassionately, and effectively.

• 📝 Conduct full assessments of patient needs (clinical, social, and lifestyle)

• 🧠 Create and manage tailored, integrated care plans

• 🤝 Coordinate with GPs, therapists, social workers & external partners

• 📈 Oversee care packages & monitor quality outcomes

• 📣 Advocate for access to appropriate treatments, medications & support

• 🏡 Support smooth transitions across care settings (e.g., hospital discharges)

• 💬 Educate patients and families in self-management & long-term care

• 📊 Analyse trends & contribute to ongoing quality improvement

• 💡 Stay up-to-date with evolving clinical guidelines and best practices

• ✅ Registered Nurse with a valid NMC PIN (LD or Paediatrics)

• ✅ Experience in Paediatrics or disability complex care, case management, or community nursing

• ✅ Compassionate, proactive and highly organised

• ✅ Strong communication and leadership skills
