Position Summary:
As a Hospital Discharge Social Worker you will play a critical role in the safe and effective discharge of adult patients from the hospital. This position involves working closely with medical staff. You will assess patient needs, develop discharge plans, and provide support and resources to promote patient recovery and well-being.
You will be a fully qualified Social Worker registered with Social Care Wales, and you will have had previous experience working within a Hospital Discharge Team role or similar.
Key Responsibilities:
Assessment and Planning:
- Conduct comprehensive assessments of patient's social, emotional, and environmental needs.
- Develop individualised discharge plans in collaboration with the patient, family, and multidisciplinary team.
- Identify appropriate community resources and support services to facilitate discharge.
Coordination and Communication:
- Act as a liaison between hospital staff, patients, families, and external agencies.
- Ensure clear communication regarding discharge plans and follow-up care.
- Coordinate transportation, home care services, and other necessary post-discharge arrangements.
Advocacy and Support:
- Advocate for patient's rights and needs within the health care system.
- Provide emotional support and counselling to patients and families during the discharge process.
- Assist patients in understanding and navigating the health care system and available resources.
Documentation and Compliance:
- Maintain accurate and timely documentation of assessments, plans, and interventions.
- Ensure compliance with hospital policies, legal requirements, and professional standards.
- Participate in regular audits and reviews to maintain high-quality discharge processes.
Collaboration and Professional Development:
- Work collaboratively with medical, nursing, and therapy staff to enhance patient care.
- Participate in multidisciplinary team meetings and case conferences.
- Engage in ongoing professional development and training opportunities to stay current with best practices and regulations.
Minimum Requirements:
-SCW Registration
-2 year post qualifying experience within UK
