As a Complex Care Nurse Case Manager, you'll take the lead in coordinating holistic, high-quality care for individuals with complex health needs. Working in a supportive, nurse-led team, you'll have the freedom to shape care packages and the support to grow your career - with pathways to senior clinical, training, or operational roles.
âš What You'll Be Doing
đ§ Carry out in-depth clinical and holistic assessments
đ Create and manage bespoke care plans
đ§© Work closely with GPs, therapists, social workers, and commissioners
đ Oversee care packages to ensure safety, satisfaction, and excellence
đŁïž Educate and empower families & patients
đ Manage transitions of care, including discharges
đ Use data to drive improvements and outcomes
đ©ââïž What We're Looking For

NMC Registered Nurse (Learning Disabilities or Children's Nursing)

Experience in complex care, community, or case management

Organised, compassionate, and confident working across teams
