Responsibilities:

• Managed a caseload of individuals with complex mental health needs, ensuring timely interventions and support.
• Conducted detailed assessments of service users' mental health, social circumstances, and risks to develop appropriate care plans.
• Developed and implemented person-centered care plans in collaboration with service users, families, and multi-disciplinary teams to promote recovery and independence.
• Identified and managed risks associated with service users, ensuring appropriate safeguarding measures and crisis plans were in place.
• Worked closely with mental health professionals, social workers, GPs, housing services, and voluntary agencies to provide holistic support.
• Provided timely interventions during periods of crisis, including liaising with Crisis Teams, inpatient units, and emergency services when required.
• Regularly reviewed and adapted care plans to reflect changing needs, ensuring the best possible outcomes for service users.
• Supported service users in accessing community resources, employment, education, and other social opportunities to enhance their quality of life.
• Ensured accurate and timely record-keeping, maintaining compliance with professional and legal requirements.
• Attended and contributed to multi-disciplinary team meetings, case reviews, and safeguarding discussions to ensure coordinated care.

Requirements:

• Social Work Degree or equivalent
• Social Work England registered
• Enhanced DBS
