Position Title: Advanced Social Worker - Family Assessment Team

Key Responsibilities:
... Comprehensive Assessments:
- Conduct in-depth assessments of families referred to the service to identify needs, risks, and protective factors.
- Gather and analyse information from various sources, including interviews, home visits, and reports from other professionals.
- Evaluate the social, psychological, and environmental factors affecting the family.
Care Planning and Intervention:
- Develop, implement, and review individualised care plans in collaboration with families and other professionals.
- Ensure that care plans are tailored to meet the specific needs of each family, promoting safety, stability, and well-being.
- Provide direct support and intervention to families in crisis.
Leadership and Mentorship:
- Provide guidance and support to less experienced social workers within the team.
- Share best practices and contribute to the professional development of colleagues.
- Lead by example, demonstrating high standards of practice and professionalism.
Multi-Agency Collaboration:
- Work closely with a range of professionals, including healthcare providers, educators, and community organisations, to coordinate comprehensive care.
- Participate in multi-agency meetings, case conferences, and strategy discussions to ensure a holistic approach to family support.
- Advocate for the rights and needs of children and families within the care system.
Documentation and Reporting:
- Maintain accurate and up-to-date records of assessments, care plans, and interventions.
- Prepare detailed reports for case reviews, care planning meetings, and other relevant processes.
- Ensure compliance with all legal, regulatory, and organisational requirements.
Qualifications:
Education:
- Fully Qualified Social Worker
Registrations:
- Fully registered with Social Work England
Experience:
- Extensive experience in family assessment, child protection, or a related field is required.
- Proven track record of effective case management and intervention skills.
- Previous experience working within a Advanced Social Worker role is essential
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
