Middlesbrough Borough Council's Locality Team is seeking a dedicated and experienced Adult Social Worker to join our team. We are committed to supporting adults in need, promoting their independence and well-being within their local community.
About the Role and Responsibilities:
- Assessment and Support Planning: Conduct comprehensive assessments of adults referred to the Locality Team, identifying their needs, strengths, and risks. Develop and implement individualised care plans, including interventions and support strategies to promote independence and well-being.
- Delivering Effective Interventions: Work collaboratively with individuals, families, and other professionals, including health services, housing providers, and the police, to deliver timely and effective support.
- Community Engagement: Actively engage with local communities, building relationships with residents, community groups, and other organizations to promote awareness of social care services and identify needs within the area.
- Advocating for Individuals: Advocate for the needs of individuals, ensuring their rights and well-being are protected.
- Caseload Management: Manage a caseload of individuals, ensuring their needs are met and that they receive the appropriate support.
- Maintaining Accurate Records: Maintain accurate records and documentation, ensuring adherence to relevant legislation and guidelines.
- Contributing to Team Development: Contribute to the ongoing development and improvement of the team's service delivery.
Key Requirements:
- Qualified Social Worker (DipSW, CQSW, BA Hons in Social Work, or equivalent).
- Social Care England registration.
- Updated DBS check.
- Experience working within Adult Social Care, particularly in a community-based setting.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving abilities.
- Ability to work effectively under pressure and manage competing demands.
- Commitment to person-centered practice and a commitment to improving the lives of vulnerable adults.
