Key Responsibilities
Assessment and Care Planning: Conduct comprehensive assessments of adults' needs, risks, and strengths. Develop and implement personalized care plans to address identified needs.
Support and Intervention: Provide practical and emotional support to adults, helping them to achieve their goals and improve their quality of life.
Safeguarding: Identify and respond to safeguarding concerns, ensuring the safety and well-being of vulnerable adults.
Collaboration: Work closely with other professionals, including healthcare providers, housing officers, and community organizations, to coordinate and deliver comprehensive support.
Advocacy: Advocate for the rights and needs of adults, ensuring their voices are heard and their choices respected.
Documentation: Maintain accurate and up-to-date records of all assessments, interventions, and decisions, ensuring compliance with relevant policies and procedures.
Qualifications and Skills
Education: A degree in Social Work and current registration with Social Work England.
Experience: Previous experience working with adults in a social work setting.
Ability to build positive relationships with adults and their families.
Strong organizational and time-management skills.
Knowledge of relevant legislation and best practices in adult social care.
Commitment to safeguarding and promoting the welfare of vulnerable adults.
