As a Complex Care Worker in this hybrid role, you will support adults with complex needs, working 2 to 3 days per week in Bexley (Civic Offices) and engaging with the local community. Your duties include comprehensive assessments, personalised care plans, and multi-agency collaboration, with a focus on person-centred care.
Responsibilities include:
... - Conduct comprehensive assessments of individuals' needs in line with the Care Act 2014.
- Develop and implement person-centred care plans, ensuring statutory compliance under the Care Act 2014.
- Collaborate with multiple agencies to ensure integrated support, following the Mental Capacity Act 2005 and Care Act 2014.
- Participate in Court of Protection work, including attending hearings if required, in accordance with the Mental Capacity Act 2005.
- Identify and address safeguarding concerns, ensuring protection under the Care Act 2014 and Mental Health Act 1983.
Requirements:
- Relevant social work qualification
- You must have 3 to 4 years post-qualification experience (including ASYE) and supervision experience
- knowledge of Court of Protection work and attendance when required
